For more than 100 years, UR Medicine Home Care has cared for patients recovering from an illness or injury in their homes. Even during a worldwide pandemic, our goal remains the same; providing our patients with the highest quality of care and assistance to help them stay safe and stay independent. UR Medicine Home Care has a full range of programs and services designed to help patients recover and stay in their homes.
We are a group of passionate caregivers and are driven to provide the best care to the community. This passion has been the basis of our past, present, and future success. A History of Innovation
We have a proud history of delivering the healing power of home through programs and services geared toward meeting our community’s home healthcare needs. We lead the way in pioneering new models of home care, creatively meeting the home-bound's nutritional needs, and providing comfort to patients and families when the end of life is near. Today we are providing services and programs in stride with our community’s needs.

Pediatric and Perinatal Services
In partnership with UR Medicine’s Golisano Children’s Hospital, our pediatric program provides comprehensive, preventive, post-acute, rehabilitative, and complex care for medically frail infants and children. Our team of experienced pediatric clinicians are dedicated to providing high-quality and compassionate care to your little one.
Arranging for Pediatric Home Care Services
- If your child is scheduled for surgery, ask your child’s doctor before being admitted to request home care from UR Medicine Home Care.
- If your child is admitted to the hospital unexpectedly, ask one of the UR Medicine Home Care Intake Specialists to review your child’s needs before being discharged. All of our specialists are registered nurses experienced in planning for home care needs.

Arranging for Perinatal Home Care Services
The UR Medicine Home Care Perinatal program serves prenatal, high-risk obstetric patients with diagnoses, including pre-eclampsia, hyperemesis gravidarum, and gestational diabetes; and ante-partum patients with wound-related diagnoses. These patients can remain on services for up to two months post-partum, and any remaining health issues after that time would be transferred to the pediatric team.
What Happens Next
A UR Medicine Home Care case manager (a nurse, physical therapist, or speech therapist) will be assigned to coordinate your child’s care at home. Working closely with your physician, your child’s case manager will oversee a complete and individualized home care plan for you.

UR Medicine Home Care Pediatrics provides care for medically complex children living in Monroe and Wayne Counties. Our team of expert pediatric clinicians are dedicated to providing the highest quality and most compassionate care in the comfort of your home.

Our expert team includes highly experienced RNs, a Pediatric Nurse Practitioner, Pediatric Social Work, Pediatric Physical, Occupational, and Speech therapists, and a Certified Child Life Specialist.
The Glover-Crask Care for Kids program provides additional therapeutic services, including Child Life and Music Therapy, to children and families in need of an extra layer of support. These will be available to our patients as part of the goal of delivering family-centered, holistic care.
Our Perinatal program offers care for women with high-risk pregnancies at home. This reduces the burden of frequent office visits necessitating transportation, child care, and potential COVID exposure, potentially resulting in a healthier pregnancy and birth.

Skilled Nursing Care
Quality care and professional service is our highest priority.
- The registered nurses at UR Medicine Home Care treat and educate patients concerning various medical conditions and provide advice and emotional support to family members. They administer treatment, medications, perform different diagnostic tests and analyze results, record patient medical histories and symptoms, and help with patient follow-up.
- For patients requiring medication treatment via intravenous infusion, highly-skilled nurses perform the latest infusion therapy techniques. They administer and monitor these treatments to ensure safe and effective delivery of medicine.
- Nurses also provide excellent care for a variety of other illness, injuries, or disorders such as wounds, ostomies, pressure ulcers, incontinence, and many other ailments.

To qualify for admission, a patient needs:
- A physician’s order
- Need for skilled and intermittent health care services
- Home bound status (only required by Medicare, other insurances may not require patients to be home bound).

Coping with an unexpected illness is stressful for both you and those who love and care for you. Our certified home health agency program is designed to meet the complex needs you may have as you learn about and how to deal with a potentially life-threatening illness.
Interdisciplinary team health care teams work closely with your primary care physician, surgeon, oncologist, and any of the regional specialists to develop an individualized home care plan for you, based on your specific health care needs.
Together, we work to maintain a patient/family focus throughout the home care process, striving to meet the medical, post-operative, psychosocial and spiritual needs of you and your family members or caregivers.
How to Receive Care
Based on your individual needs, your physician will make a home care referral. You may inform your hospital nurse or physician about your preference in using UR Medicine Home Care. Referrals are accepted seven days a week by calling us at (800) 253-4439.
Insurance Coverage
The cost of services may be covered by Medicaid, Medicare, HMO’s and private insurance companies and will be verified before admission.
